    Perspective Application Form

    Please ensure all employment history dates are provided within the 6-year period.

    FCA Long Form A

    We will be using this to submit your FCA Application.

    Contact details for references

    If known confirm appointed rep details (general company email if no direct).

    Employment Gaps

    If there are any gaps in your employment history, please provide an explanation of what you were during this time and how you were financially supporting yourself.

    CRB Check started

    ID Documents

    Provided a copy of your ID Documents. We will need to receive a copy of your Passport and Drivers Licence.

    Proof of Address

    Provide copy of proof of address, details of which are given in the ID section.

    Learning Statements

    Provide a copy of your learning statement. You will be able to obtain this from your examining body.

    Qualifications and Certificates

    Provide copies of the Qualifications that you have received.

    Copy of Statement of Professional Standing (SPS)

    Credit Report

    Provide a copy of your Credit Report. This can be obtained from the Experian or Equifax Website. You can sign up with either of these websites to obtain a free copy (Please note, we will not be able to accept a picture of your credit score).

    Confirmation of post nominals e.g FPFS.

    Documentation to confirm use of post nominals

    FCA Accredited Body Membership Details

    Provide the following information which can be obtained from your FCA Accredited body. Name of Accredited Body, Membership Number and Renewal Date.

    Sign consent to carry out ongoing DVLA check
